Training Supply Information

To complete webcam proficiency training, you must have:
- 6–12 DOT-approved Oral Fluid (split) Collection Kits
- Federal Custody and Control Forms (CCFs) from your lab

Don’t have the required supplies? We can provide them at an additional fee of $80 per student. Supplies ship only after full payment.
